Between 1985 and 1996, 25 young patients (pts) with myelodysplastic syndrome (MDS) were treated in Fundeni Hospital; mean age 28.5 years (range 16-40 years); sex: 15 female, 10 male. All pts
were classified according to FAB proposals: refractory anemia (RA) 13 (52%) cases, refractory anemia with ring sideroblasts (RAS) 3 cases (12%), refractory anemia with excess of
blasts(RAEB) 4 cases (16%) refractory anemia with excess of blasts in transformation(RAEB-t) 5 cases (20%). The pts were divided into two groups: low-risk pts (RA and RAS, No=16) and
high-risk pts (RAEB, RAEB-t, No=9). Pts of low-risk received transfusions, hormones (cortico-steroids, androgens), while the other group received LDAC. Pts with RAEB may benefit from the
treatment with LDAC, and those who respond to a single course and relapse are more likely to benefit from retreatment. From the low-risk group, 10 pts (62%) are alive and maintain the
remission with supportive treatment; from the high-risk group only 3 (33%) pts are alive. The median survival was 18 mo (range 3 - 67 mo). The two years survival probability for low-risk
group was 46±10% and for high-risk group was 15±10%. Risk factors for the treatment failure were: number of blasts in bone marrow, low colony formation and chromosomal abnormalities. DAC
